Qualificações

• Minimum 1–2 years of experience in social services or casework management.
• Strong knowledge of Singapore's social service network and government subsidies.
• Excellent case-documentation and PDPA confidentiality
• Comfort using client data management portals (EPES or similar).

Responsabilidades

• Practical case management and resource support to remove barriers to recovery.
• Intake, triage and administrative screening for referrals; route to counselling team.
• Client retention and care coordination; re-engagement with clients missing sessions.
• Multi-agency liaison with social service agencies and hospital networks.
