Job summary

Tan Tock Seng Hospital is seeking a Program Coordinator for the Stroke Rehabilitation Ecosystem in Singapore. The role supports operations, coordination, implementation, and evaluation of the ecosystem under the Programme Clinical Leads.

Qualifications

• Diploma or Degree required or equivalent.
• Background in Nursing, Medical Social Work, Counselling, Psychology or Ethics preferred.
• Independent with initiative and strong interpersonal skills are essential.
• Excellent communication skills in English are required.
• Meticulous and well-organised with good attention to detail.
• Proficient in Microsoft Office (Word, Excel, PowerPoint).
• Experience in acute/intermediate and long-term healthcare is advantageous.

Responsibilities

• Review daily stroke admissions to identify eligible patients.
• Collate and track patient progress data and assist analysis.
• Provide basic psychological interventions.
• Redirect patients to appropriate service tiers as needed.
• Maintain relationships with key agencies and community partners.
• Implement workflows to ensure seamless referrals and operations.
• Assist in planning, implementation, and management of activities/projects to improve service quality and patient care.
• Contribute to enhancement of the ecosystem and its component services.
• Arrange and support meetings with internal and external stakeholders.
• Support progress updates to funding organisations.
• Participate in department, division, and hospital initiatives as needed.
• Handle any other duties as assigned by HOD/Supervisor or Programme Leads.

About the Programme

The Stroke Rehabilitation Ecosystem aims to enhance the quality of care for stroke survivors in Singapore. With stroke being a leading cause of death and disability, this program seeks to bridge gaps in rehabilitation services, ensuring a seamless transition from hospital to community care.

Key Responsibilities

The Program Coordinator will support the operations of Stroke Rehabilitation Ecosystem, with the leadership of the Program Clinical Leads in the coordination, implementation and evaluation works related to the ecosystem. This includes:

• Review daily stroke admissions to identify and engage patients eligible for the program.
• Collate and track data on patient’s progress and assist with analysis of data.
• Provide basic psychological interventions.
• Assist to redirect patients to suitable service tiers should patients clinically deteriorate.
• Ensure relationships with key agencies and community partners are maintained.
• Implement workflows to ensure seamless referral and operations.
• Assist in planning, implementation, and management of activities and projects that contribute towards service/quality/process improvement and patient care.
• Assist in enhancement of service and quality of the ecosystem and its component services.
• Arrange and support relevant meetings with internal, external stakeholders and partners.
• Support progress updates to funding organisation.
• Participates actively as a member in department, division, and wider hospital initiatives.
• Any other matters as assigned by HOD/Supervisor and/or Program Clinical Leads.
