Job Role Summary:
Provide comprehensive assessment, and recommendations to the care team and/or caregivers on implications of cognitive, physical and social impairment on ADL and IADL engagement for persons with dementia
Provide recommendations on activity prescriptions for persons with dementia
Provide job training and support to staff and/or caregivers on strategies in engaging persons at various stages of dementia, in individual, group sessions or other specific programmes (e.g. THOR and PEACE)
Review staff competency levels in activity facilitation for persons at various stages of dementia
Plan group activities programmes in day care and make necessary modifications to cater programme schedule to suit individual centre’s needs
Provide recommendations on environmental modification in the daycare or home setting to maximize client’s participation in ADLs and IADLs
Provide comprehensive assessment in understanding the behaviour of concerns of persons with dementia and advice on appropriate recommendations and strategies to staff and or caregivers
Provide support and training to staff on implementation of strategies in managing behaviour of concerns at the centre level
Document professional input and recommendations on electronic documentation and communicate to relevant care staff and/or caregivers
Develop training and educational materials. Conduct training/sharing to direct care team and public event
Provide a comprehensive and holistic assessment to identify needs, coordinate services, educate, advocate and empower clients and their support systems with the aim of enabling the clients to remain in the appropriate care setting and achieving optimal care
Coordinates the process, and collaborates with the client, caregivers and key service providers to ensure that the plan isdeveloped appropriately
Formulate a care plan and coordinate care for the individual and their families, identify appropriate providers and facilities throughout the continuum of services, ensuring that available resources are being used in a timely and cost- effective manner
Facilitate or lead centre interdisciplinary case discussion within senior daycare or other community partners
Develop and review programmes and evaluate their effectiveness and efficiency to contribute to continuous program improvement
Provide consultancy in centre design layout and environmental modification to be dementia friendly environment
You should:
Registered Nurse registered with Singapore Nursing Board
Attained Advanced Diploma in Gerontology/Medical & Surgical/Mental health or in any discipline / Specialist Diploma in Geriatric / Community Nursing
Attained Diploma/Bachelor of Science in Nursing
With at least 5 years of experience in the area of mental health, geriatric, or dementia populations in any settings
Occupational Therapist registered with Allied Health Professional Councils
Attained Diploma/Bachelor of Science in Occupational Therapy
With at least 5 years of experience in the area of mental health, geriatric, or dementia populations in any settings
