Job summary

NTUC Health Co-operative Limited is seeking a Practitioner in Dementia Care to deliver comprehensive assessments, care planning, and staff training for persons with dementia across NTUC Health’s eldercare services.

The role includes coordinating with caregivers, developing activity plans, and advising on environmental modifications to empower clients to remain in appropriate care settings.

Qualifications

• Registered nurse with SNB registration.
• Advanced Diploma in Gerontology/Medical & Surgical/Mental Health or related.
• Diploma or Bachelor of Science in Nursing.
• Occupational Therapist registered with Allied Health Professional Councils.
• Minimum 5 years of experience in mental health, geriatrics or dementia care.

Responsibilities

• Provide comprehensive assessment and recommendations to the care team on cognitive, physical and social impairment on ADL/IADL for persons with dementia.
• Provide recommendations on activity prescriptions for persons with dementia.
• Provide staff training and support on engagement strategies for persons at various dementia stages.
• Review staff competency in activity facilitation for dementia.
• Plan group activities in day care and modify schedules to centre needs.
• Advise on environmental modification to maximise participation in ADLs/IADLs.
• Coordinate care with clients, caregivers and service providers to develop care plans.
• Develop and deliver training materials and conduct sessions for care teams and public events.
