Job summary

Qualifications

• Degree or Master in Pharmacy is required or expected.
• Registered or eligible for registration with the Singapore Pharmacy Council (SPC).
• Strong communication and interpersonal skills.
• Team player with a passion for patient-centered care.
• Comfortable with rotating shifts.

Responsibilities

• Review medication orders for appropriateness and safe dispensing.
• Provide medication counseling to patients for safe use.
• Deliver drug information and clinical advice to doctors, nurses, and patients.
• Support training and supervision of pharmacy assistants.
• Perform quality assurance activities including ward inspections and expiry checks.
• Manage medication-therapy duties like supply and distribution of controlled drugs and ADR reporting.
• Participate in medication reconciliation rounds and specialized counseling (e.g., warfarin, inhalers).
• Contribute to ICU, transplant, pain management, and diabetes care where applicable.

Key Responsibilities

• Review medication orders for appropriateness and ensure safe, accurate dispensing of medication
• Provide medication counseling to patients to support safe and effective use
• Deliver drug information and clinical advice to doctors, nurses, and patients
• Support training and supervision of pharmacy assistants
• Perform quality assurance activities such as ward inspections and expiry check
• Manage other medication-therapy related duties, such as supply and distribution of controlled drugs, adverse drug reactions reporting, drug usage monitoring
• Participate in medication reconciliation rounds and provide specialised counseling (e.g. warfarin, inhaler techniques)
• Contribute to specialised care areas such as ICU, transplant, pain management, and diabetes care (where applicable)
• Take part in projects and improvement initiatives as assigned
