Job summary

Qualifications

• Bachelor of Science degree in Pharmacy from an accredited University recognized by the Singapore Pharmacy Council.
• Minimum 2 years Pharmacist experience.
• Prior experience at public health institution.
• Registered with the Singapore Pharmacy Council.
• Valid practicing license.

Responsibilities

• Provides pharmaceutical services in inpatient wards including ICU, with medication reconciliation and timely verification of supplies and discharges.
• Participates in multidisciplinary rounds, advising ICU physicians on pharmacotherapy.
• Trains Pharmacists and pre-registration Pharmacists in medication review.
• Educates patients on pharmacological and non-pharmacological aspects of therapy and promotes adherence.
• Covers central discharge pharmacy on a rotational schedule to ensure timely and safe dispensing of discharge medications.
• Ensures all sales and supply of medicinal products comply with legal requirements.
• Maintains records pertaining to drug receipt, storage and distribution as required by hospital/regulatory authorities.
• Performs inspections of assigned drug storage areas in the hospital.
• Provides drug information to patients and healthcare professionals.
• Implements and adheres to policies, practices and regulations set by the department, hospital and external agencies.

JOB PURPOSE

This job description details the responsibilities and the general duties of the Pharmacist (Inpatient and Intensive Care Unit (ICU)).

MAJOR DUTIES & RESPONSIBILITIES

Specific

• Provides pharmaceutical services in WH inpatient acute wards, including ICU. This includes medication reconciliation, timely verification of supplies and discharges for the allocated ward.
• Participates in multidisciplinary rounds of ICU/acute wards, providing input to ICU physicians on pharmacotherapy. In-house training for ICU coverage can be arranged if needed.
• Trains Pharmacists and pre-registration Pharmacists to do medication review for these services through a structured framework.
• Educates patient on the pharmacological and non-pharmacological aspects of their therapy. Checks compliance and encourages adherence to medication therapy where applicable.
• Covers the central discharge pharmacy on a rotational schedule, providing floor support and patient care to ensure timely and safe dispensing of discharge medications.
• Ensures all sales and supply of medicinal products comply with legal requirements.
• Ensures records pertaining to all phases of drug receipt, supply, storage and distribution are maintained as required by the hospital and/or legal authorities.
• Performs inspections of assigned drug storage areas in the hospital.
• Provides drug information to patients and other healthcare professionals.
• Responsible for the implementation and adherence to policies, practices and regulations established by the department, hospital and external regulatory agencies pertaining to pharmacy.

• Conduct clinical review for appropriateness of medication orders.
• Ensure operational efficiency to meet desired turnaround time.
• Supervise performance and conduct appraisal.
• Support and actively participate in various activities organised by hospital, such as QC, community projects, staff welfare and recreational activities etc.

JOB REQUIREMENTS

Education / Training / Experience
