Job summary

Dementia Singapore Ltd in Singapore is seeking a Case Work Support Officer to assist caseworkers with administrative tasks and client-facing duties. You will liaise with hospitals and social services, provide psychoeducation and basic counselling, help run outreach activities, and maintain timely reports and records while traveling islandwide for home visits as required.

Diploma in social work or related field preferred; 2–3 years healthcare/community care experience advantageous; strong

Qualifications

• Diploma in social work, counselling, psychology or related field.
• 2-3 years healthcare/community care experience preferred.
• Meticulous with details.
• Proficient in Microsoft Office and administrative tasks.

Responsibilities

• Provide administrative support to caseworkers.
• Support casework tasks including information and referrals, liaising with external agencies, and home visits.
• Liaise with hospitals and social services to triage services for people with dementia and their carers.
• Provide basic psychoeducation and supportive counselling as part of case management.
• Assist in running programmes and outreach activities.
• Partake in network and collaborative sessions with external partners.
• Prepare minutes of meeting.
• Keep appropriate case recordings and produce timely reports; data entry and filings.
• Perform other duties as required.
