Job summary

Recruit Express Pte Ltd is seeking a dedicated nurse for home-based palliative and chronic illness care on the East side of Singapore. You will perform home visits, assess patients, and coordinate with doctors for follow-up visits.

The role includes wound care, catheter care, NG tube management, and caregiver education to ensure continuity of care. Permanent staff enjoy a basic salary up to SGD 3,900 monthly, with locum allowances per case.

Qualifications

• Palliative care training or homecare experience required.
• SNB registration and BCLS certification required.
• Working location: East side; transportation covered by company.

Responsibilities

• Perform home visits to chronic illness and palliative care patients who have been referred.
• Conduct nursing assessments on patients.
• Coordinate with doctors for medical assessments and follow-up visits.
• Monitor patients’ current medication and compliance with doctors’ prescriptions.
• Perform nursing procedures such as wound dressing, urinary catheter insertion, nasogastric tube insertion and care of tubings, as required.
• Initiate nursing care plans and implement new management measures.
• Educate caregivers and family members on basic nursing procedures to ensure continuity of care at home.
