Job summary

Recruit Express Pte Ltd seeks a caregiver/nurse to perform home visits for chronic and palliative patients in Singapore, conducting thorough nursing assessments and coordinating with physicians for ongoing care. You will monitor medications, perform wound care, catheter and tube care, and initiate care plans as needed.

You will also educate families on essential home care procedures to ensure continuity of care and patient comfort.

Qualifications

• Palliative-trained or home care experience is required.
• Valid SNB registration and BCLS certification are mandatory.
• IT Savvy with basic system usage is necessary.

Responsibilities

• Perform home visits to chronic ill/palliative patients referred.
• Conduct nursing assessments during visits.
• Coordinate with doctors for medical assessments and follow-up visits.
• Monitor medications and patient adherence as prescribed.
• Carry out nursing procedures such as wound dressing and catheter/NGT care.
• Initiate and implement nursing care plans and new management measures.
• Educate caregivers/families on basic nursing procedures for home care.
