Job summary

Responsibilities include handling inquiries, coordinating assessments and financial counselling, and ensuring accurate documentation and records. 2+ years in a nursing home setting is preferred, with strong MS Office and HCSIS skills.

Qualifications

• Diploma in Social Sciences, Social Work, Psychology, or related discipline.
• At least 2 years’ relevant experience in a nursing home or long-term care setting.
• Experience in admissions and resident administration.
• Proficiency in MS Office and healthcare information systems.
• Strong organisational, communication and interpersonal skills.

Responsibilities

• Support the MSW in end-to-end admission and discharge process.
• Attend admission enquiries from residents, family members, hospitals and external stakeholders.
• Coordinate nursing assessments, financial counselling, and admission arrangements.
• Prepare and process admission, discharge and resident documentation. Maintain accurate resident records and ensure timely updates.
• Assist with applications for financial assistance and government subsidies.
• Follow up on outstanding payments and billing-related matters.
• Liaise with internal departments and external agencies to ensure smooth coordination.
• Support departmental KPIs and perform any other duties assigned by the MSW or Management.
