Job summary

SAMH Group Homes is seeking a dedicated Case Worker to support residents in an independent living facility for people with mental health conditions. You will assist with intake, admission, orientation, and ongoing case management within a team-driven environment.

You will collaborate with Ops and other SAMH programmes, help organise outreach activities, prepare documents, and ensure residents' wellbeing and progress toward rehabilitation and community integration.

Qualifications

• ITE or A level. Diploma, preferably in Social Work, Community Services, Psychology, Counselling or Healthcare-related would be an advantage.
• Preferably some experience working in a social service setting in a direct work role. Experience in a residential facility or a mental health setting will be an advantage.
• Experience with coordination and administrative functions.
• Adaptable with the ability to flow with changes when working with residents.
• Good energy level and willingness to work beyond office hours, if required
• Good interpersonal and communication skills

Responsibilities

• Assist Case Workers in the preparation of intake interviews for new referrals.
• Assist Case Workers with orientation visits during intake interviews.
• Work with Ops Team to prepare room and allotted cabinets for new admissions.
• Assist Case Workers in the admission of new residents into the programme, eg. Orientation and ensure linen belongings are placed in dryer.
• Assist Case Workers in preparing documents required by residents for their appointments.
• Assist Case Workers in sourcing and applying resources and services required by residents.
• Assist in handling requests and grievances of residents.
• Assist Case Workers in monitoring implemented intervention plans for residents.
• Attend regular staff meetings and participate in case discussions when required.
• Compile updated records on residents' particulars and statistics for monthly reports to meet funders' requirements.
• Support Programme Head and team in administrative tasks.
• Orientate and host visitors to SAMH centres.
• Increase public awareness of mental health through outreach events.
