Banyan Home seeks a committed Case Worker to support residents in a long-term psychiatric home for adults facing homelessness or severe hardship. You will help residents stabilize, plan their care, and work toward recovery and reintegration with dignity and autonomy.

Key responsibilities include assessments, individualized care planning, coordination with healthcare and housing services, advocacy for public assistance, and crisis de-escalation as needed.

• Conduct monthly assessments and interviews with residents on their care needs and rehabilitation plans.
• Develop individualized care plans in collaboration with residents and care staff.
• Prepare reports and present residents' cases to stakeholders.
• Coordinate comprehensive care with hospitals, mental health services, housing referrals, and other organizations.
• Advocate for resident access to public assistance, healthcare, housing and vocational services.
• Provide psychoeducation, counseling, and skills coaching on daily living, budgeting, and medication adherence.
• Monitor progress and update care plans as needed.
• Respond to behavioral health crises using de-escalation and medical approaches.
• Support transitions to community housing when possible.

We are seeking a committed and compassionate Case Worker to support residents in along-term psychiatric home serving adults with psychiatric issues who are also experiencing homelessness or extreme socioeconomic hardship. This role is essential in helping residents stabilize, manage their long-term care and work towards recovery and reintegration into the community, while ensuring dignity, respect, and autonomy.

Key Responsibilities:

• Conduct monthly assessments and interviews with residents on their care needs and rehabilitation plans.
• Develop individualized care plans in collaboration with residents, Banyan Home nursing and care officers, and external stakeholders.
• Preparation of reports and presentation of resident’s case to relevant stakeholders.
• Coordinate comprehensive care including through hospitals, mental health services, substance use treatment, housing referrals, employment entities and other relevant organizations.
• Advocate for resident access to public assistance, legal support, healthcare, housing and vocational services.
• Provide individual or group psycho education, supportive counseling, and skills coaching focused on daily living, hygiene, budgeting, and medication adherence.
• Monitor resident progress, document care activities, and update care plans as needed.
• Respond to behavioral health crises using de-escalation and appropriate medical approaches.
• Support residents in preparing for community transitions, including supportive or independent housing when possible.
• Maintain ethical standards, cultural sensitivity, and confidentiality at all times.
• Perform any other duties as assigned by the supervisor or program leadership that are consistent with the role and support the overall mission of Banyan Home.
