Job summary

Qualifications

• Minimum 4 years of experience in managing mental health or related settings.
• Strong communication and documentation skills in clinical settings.
• Ability to work with a multidisciplinary team and external partners.

Responsibilities

• Assess, plan, coordinate, monitor and track patient care to ensure continuity of care.
• Collaborate with doctors, psychologists, nurses, social workers and allied health staff.
• Provide timely documentation in patient records and care databases.
• Engage with community partners and coordinate discharge planning and transitional care.
• Assist in maintaining KPIs and reporting for care programs.
