Responsabilités

• Provide clinical and psychosocial assessments and goal planning.
• Screen and monitor patients' mental status and treatment plans.
• Counsel, educate, and advocate for patients and families.
• Coordinate discharge planning and home visits as needed.
• Collaborate with a multidisciplinary team across hospital services.
• Maintain patient records and contribute to clinical indicators reports.
• Assist in research and literature reviews for project submissions.
