Job summary

National University Hospital in Singapore seeks a Case Management Officer to provide a continuum of psychological care for women affected by mental disorders. You will conduct assessments, plan individualized treatment, and coordinate services with a multidisciplinary team.

The role involves home assessments, home visits, and liaison with O&G and community resources, ensuring timely access to care and ongoing support until discharge.

Qualifications

• Psychology degree preferred with honors.
• Experience in social/mental health settings is advantageous.
• Computer literacy including MS Office is required.

Responsibilities

• Provide clinical and psychosocial assessments, determine diagnosis and goals.
• Screen and monitor patients' mental status.
• Provide counseling, psycho-education and advocacy for patients and families.
• Assess symptoms, monitor medication adherence and provide relapse prevention advice; crisis intervention when needed.
• Coordinate discharge planning and formulate care plans with clinicians.
• Coordinate with multiple disciplines to ensure timely tests, treatments and consultations.
• Arrange and conduct home visits with patients and families, and provide home-based interventions.
