Job summary

Stafflink Services Pte Ltd is recruiting a Medical Social Worker for a leading healthcare provider in Singapore. The role focuses on socio-economic evaluations, family counselling, and discharge planning within a hospital setting.

You will collaborate with medical staff and community services to achieve optimal end-of-life goals and provide bereavement support to families. Applicants should have a social work degree and relevant registrations, with at least 3 years in medical social work.

Qualifications

• Degree in Social Work-related disciplines with relevant registration.
• Masters in Counselling or related field is preferred.
• Minimum 3 years of medical social work experience in healthcare setting.

Responsibilities

• Provide socioeconomic evaluations to assist patients and families with end-of-life practical concerns.
• Conduct ongoing individual and family counselling via in-person or home visits.
• Collaborate with hospital teams and community organizations to support end-of-life goals.
• Engage families to encourage visits and prepare for life closure and dying process.
• Offer bereavement information and support to families of deceased patients.
• Attend Interdisciplinary Patient Care Conferences with medical and nursing staff.
• Counsel patients and families on discharge planning and financial arrangements.
• Perform field work visiting patients in hospital wards and social service agencies.
• Prepare social reports for management and external agencies.
• Present cases for review in department or inter-department meetings.
• Participate in in-service education to transfer learning.

• Prior exp with medical social work in hospital setting is required!

Our client is a leading Healthcare provider

Responsibilities:

• Provide and facilitate socio-economic evaluations to help patients and their families address practical concerns, such as financial matters, which are often an issue at the end of life
• On-going individual and family counselling through face-to-face sessions or home visitations to help navigate the personal and ethical dilemma, cultural conflict and issue relating to religion, spirituality and the meaning of life
• Work with the broader community which includes educational, religious, and social service organizations so that patients and their families can achieve optimal end of life goals
• Engage families’ involvement and encourages their visits, as well as prepares patients / families for the end-of-life stage and managing the life closure and the dying process
• Provide bereavement information, education and support to help the families of the deceased patients cope with their grief and loss
• Attend Interdisciplinary Patient Care Conferences with medical, nursing, allied health colleagues to discuss plans for total management of the patient
• Counsel patients and their family members on financial and discharge care plans
• Perform field work involving visiting patients in hospital wards and other social agencies to help patients alleviate or resolve their medico-social problems
• Prepare social and other related reports for management and other related institutions / agencies
• Present cases for discussion and engages actively on formal platforms such as departmental or inter-departmental meetings for the purpose of sharing, reviewing and clarifying of atypical, difficult issues
• Participate in in-service education programme and follow-up to ensure optimum transfer of learning
• Any other duties as and when assigned

Requirements:

• Degree in Social Work-related/Masters in Counselling related disciplines
• Registered with the Singapore Association of Social Workers/Singapore Association for Counselling
• At least 3 years of medical social work experience in healthcare institution or hospital setting
• Able to communicate in local dialects is desirable

Other Information:

• Working Days/Hours: Mon-Fri, 8am to 530pm / Fri ends at 5pm
• Location: Near Marymount MRT (Transport is provided)
