Job summary

THK Early Intervention for Children Services is seeking a Social Work Associate to support families with children under seven years old who have developmental needs in Tampines. You will conduct intake assessments, perform home visits, and collaborate with a multidisciplinary team to design and implement holistic care plans.

The role requires a Diploma in Social Work and 1–3 years of relevant experience, with strong administrative and communication skills.

Qualifications

• Diploma in Social Work required or equivalent
• 1–3 years of experience with families and children preferred
• Strong communication and documentation skills

Responsibilities

• Case management for children and families
• Explain programmes to parents and conduct intake assessments using BPS framework
• Build rapport with parents and perform RBC during home visits
• Identify follow-up needs and refer to internal/external resources
• Collaborate in multi-disciplinary team meetings with teachers, psychologists and therapists
• Contribute to holistic intervention planning and service delivery
• Handle social work administrative tasks and maintain records
• Track government Means Test applications and ensure timely renewals
• Coordinate with community agencies to ensure integrated support for clients
