Job summary

SPD in Singapore seeks a Social Worker for the Early Intervention Programme for Infants and Children (EIPIC). You will deliver social work intervention and comprehensive case management, collaborating with a transdisciplinary team and partners in EI and disability sectors to support children and families.

Responsibilities include intake, needs assessments, transition planning, and referrals to SPED or mainstream schools, with a focus on trauma-informed, family-centred practice and documentation.

Qualifications

• Bachelor of Social Work from a recognised institution.
• Registered with the Singapore Association of Social Workers (RSW status).
• Experience in direct social work practice, preferably in the disability or community-based sector.
• Strong analytical and clinical reasoning skills.
• Adaptable and resilient, with the ability to work independently and in a team.
• Proficient in professional documentation and report writing.

Responsibilities

• Handle intake and needs assessments.
• Guide families through the orientation process for the programme.
• Conduct biopsychosocial assessments and formulate intervention plans.
• Manage a caseload across varying risk levels with clinical judgment.
• Coordinate referrals to services and community resources.
• Provide trauma-informed, person-centred support to clients and caregivers.
• Participate in transdisciplinary team meetings and joint care planning.
• Maintain accurate documentation and reporting.
• Support transition planning and engagement with schools.
• Engage in supervision and professional development.
