Job summary

TOUCH Community Services supports Occupational Therapists and Physiotherapists in delivering home therapy services at clients' residences and the centre in Ang Mo Kio. The role involves assisting therapists with treatment plans, screenings, client referrals, and timely communication with the therapists in charge.

Attention to infection control, accurate record-keeping, and collaboration with the care team are essential. Experience in health care settings and proficiency in MS Office are required.

Qualifications

• 2-3 years of working experience in the health care or community service sector.
• Proficient in MS Word, PowerPoint, Excel and Outlook.
• Familiar with social and health care resources.

Responsibilities

• Assist therapists in implementing treatment plan for routine cases as directed by therapists.
• Support therapists to screen for falls risk factors, caregiver burden and other needs.
• Support in the care of clients by referring them to other care services under the direction of therapists.
• Provide timely feedback about client and/or caregiver to therapists in-charge.
• Maintain timely and accurate documentation of observations made during sessions about client and/or caregiver.
• Participate and support in quality improvement project, research or any new initiatives as assigned.
• Participate in in-house trainings and department meetings.
• Maintain inventory and equipment used for therapeutic activities during home therapy and at the centre.
• Comply and support in infection control procedures during home visits.
• Support in scheduling appointments for clients if required.
