Department: Case Management Unit

Job Description

To provide leadership and strategic direction for Case Management Unit ensuring the delivery of high-quality clinical services aligned with hospital mission, core values and strategic objectives. The Head of Department will lead departmental transformation, manage resources effectively, and foster professional development whilst maintaining excellence in patient care and service delivery.

Job Responsibilities
CLINICAL SERVICES AND STRATEGIC LEADERSHIP

Align departmental mission and objectives with hospital strategic goals, developing long-term strategic plans for service delivery and clinical excellence within the Allied Health discipline.

Establish, implement and review departmental policies, procedures and clinical practice standards to ensure provision of highest quality healthcare services in compliance with professional regulatory requirements.

Lead evaluation and modification of clinical service delivery models, incorporating evidence-based practices and innovative approaches to meet evolving patient needs and healthcare transformation initiatives.

Monitor achievement of departmental key performance indicators including clinical outcomes, service quality metrics, and operational targets in alignment with hospital strategic objectives.

Keep abreast of latest developments in Case Management and possibly recommend introduction of new services, treatment modalities and initiatives to enhance patient care and departmental capabilities.

HUMAN RESOURCE MANAGEMENT

Conduct recruitment, manpower planning and retention strategies to ensure optimal staffing levels and skill mix within the department to meet service demands effectively.

Provides supervision, guidance and performance management for all departmental staff including Case Managers, Case Trackers and Admin Support Staff through regular appraisals and development planning.

Plan and implement succession planning initiatives, identifying and developing future leaders within the department whilst ensuring knowledge transfer and continuity of expertise.

Cultivate a culture of learning by establishing relevant educational programmes, in-service training and continuing professional development opportunities to maintain current competencies and meet strategic organisational needs.

Ensure optimal staff deployment across various clinical areas and services to maximise productivity, efficiency and quality of patient care delivery.

Cultivate One NHG culture through exemplifying One NHG Cultural Beliefs and committing to staff engagement efforts to build an inspired and engaged workforce.

RISK, SERVICE QUALITY AND IMPROVEMENT

Manage departmental service quality by establishing service standards, identifying customer needs, improving workflows and building a culture of excellence and continuous improvement.

Lead hospital quality systems and initiatives including patient safety programmes, clinical governance frameworks e.g. compliance to ESS standards and safety regulations, and quality improvement projects at departmental and hospital-wide levels.

Oversee Enterprise Risk Management and implementation of mitigation plans including BCP.

Implement mechanisms to monitor clinical practices and standards, utilising patient feedback, outcome measures and performance data to drive service improvements and innovation.

Respond effectively to clinical complaints and feedback from patients, families and internal customers, implementing corrective actions and preventive measures as required.

Develop multi-disciplinary approaches to patient care through enhanced inter-departmental coordination and cooperation, fostering integrated care delivery models.

FINANCE AND RESOURCE MANAGEMENT

Work collaboratively with operations teams to provide financial accountability through forecasting, preparing and justifying operational and capital budgets for departmental requirements.

Review and manage departmental budgets and resources, monitoring variances and implementing cost-effective resource utilisation strategies whilst maintaining service quality and clinical outcomes.

Implement strategic plans and initiatives for resource development and allocation, ensuring optimal utilisation of equipment, facilities and human resources to support departmental objectives.

EDUCATION, RESEARCH AND PROFESSIONAL DEVELOPMENT

Establish and develop quality improvement projects that advance clinical practice, contribute to evidence-based care and enhance the reputation of the department and profession.

Develop educational and training materials for tertiary students, medical staff, nurses and other healthcare professionals including external stakeholders and community education initiatives.

Identify individual staff learning needs and facilitate professional development opportunities aligned with clinical excellence and organisational strategic directions.

Represent the department in clinical and professional committees, conferences, workshops and external professional bodies to maintain currency and contribute to professional advancement.
