Qualifications

• Bachelor degree or equivalent in social work or related field.
• SWAAB registration as a social worker.
• 3–5 years of relevant experience, hospital or community hospital preferred.
• Experience working with elderly and dementia care.

Responsibilities

• Intake screening with nursing staff and psychosocial assessments.
• Provide counselling and ongoing case management for residents and families.
• Coordinate referrals to community resources and government agencies.
• Facilitate Advance Care Planning discussions and documentation.
• Develop discharge and transition plans with the team.
• Coordinate respite care referrals and placement planning.
• Conduct risk assessments and crisis interventions as needed.
• Provide bereavement support and coordinate related services.
• Maintain records, reports and participate in quality initiatives.
• Engage residents in programmes and participate in multidisciplinary meetings.
