Qualifications

• Degree in Social Work from a recognised institution.
• Experience in healthcare, eldercare, nursing homes or community social services is advantageous.
• Strong counselling, communication, assessment and case-management skills.
• Good knowledge of Singapore's healthcare and social-service resources.
• Compassionate, empathetic and able to work effectively with elderly residents and their families.
• Able to work independently as well as within a multidisciplinary team.

Responsibilities

• Conduct psychosocial assessments and create individualized care plans.
• Provide counselling and emotional support to residents and families.
• Assist with financial assistance and social-service resources.
• Facilitate family meetings and care planning discussions.
• Coordinate with hospitals, community agencies, and providers.
• Support admission/discharge planning and continuity of care.
• Identify and manage psychosocial, family, safeguarding and end-of-life concerns.
• Maintain case documentation and confidentiality.
• Collaborate with the multidisciplinary healthcare team.
• Participate in service improvement projects.
• Liaise for donations and manage volunteers.
