Qualifications

• Strong interpersonal and problem-solving skills.

Responsibilities

• Admissions: screen referrals and assess suitability with nursing teams and stakeholders.
• Casework and clinical interventions with the multi-disciplinary team and residents’ families.
• Financial assistance: assess subsidies and means-tested schemes for residents.
• Reports and admin: keep resident profiles and social reports updated, coordinate with Finance.
• Supervise and support the case management team to ensure smooth operations.
