Qualifications

Responsibilities

Duties & Responsibilities

• Assess the strength and resilience of the resident, families, and social support systems to help the individual function within the community
• Co-ordinate social services through a case management approach
• Identify and refer cases for financial assistance
• Financial counselling
• Address psychosocial needs through care and counselling
• Educate resident’s family member on the physical and psychosocial needs and mediate familial conflicts
• Assist staff members in understanding the significant social and emotional factors related to the residents’ health problems
• Assess risk of self-harm and to others
• Work with nursing and other allied health professionals, residents and their families to develop and implement the care plan and the post discharge care plan
• Discuss long-term care issues with the resident and family
• Train other staff members on communication skills, mediation skills, stress management etc
• Other duties assigned by the Management

Requirements

• Degree/Post Graduate Diploma in Social Work with relevant experience
