Job summary

Raffles Medical Group Ltd is seeking an Executive / Senior Executive in Population Health & Digital Transformation to drive initiatives improving chronic disease outcomes through population health management and technology-enabled solutions.

You will identify care gaps, engage clinicians and stakeholders, and develop digital tools to enhance patient care and clinical outcomes, coordinating with IT partners and clinics.

Qualifications

• Degree in a healthcare-related field (Public Health, Healthcare Management, Nursing, Pharmacy, Life Sciences).
• Experience in healthcare operations, population health, quality improvement, or healthcare analytics.
• Comfortable with data and engaging with healthcare stakeholders.
• Strong communication, analytical and stakeholder management skills.
• Organised and able to manage multiple projects and priorities.
• Experience with clinicians or healthcare technology projects is a plus.

Responsibilities

• Analyse healthcare and patient data to identify care gaps, trends and opportunities for improvement.
• Work with doctors and clinic teams to support population health and quality improvement initiatives.
• Coordinate and monitor healthcare programmes, including tracking outcomes and progress.
• Support the development and implementation of digital health tools for patients and clinicians.
• Work with internal teams, IT and external vendors on system requirements, testing and implementation.
• Prepare reports, programme updates and recommendations for management.
• Support other population health and chronic disease initiatives as required.
