Job summary

SUN-DAC is seeking a Case Manager to coordinate person-centred case management services for clients and their caregivers in Singapore. The role oversees admissions, care planning, multidisciplinary coordination and collaboration with internal and external stakeholders to ensure holistic care.

Responsibilities include conducting psychosocial assessments, developing care plans, reviewing ISP with the team, and providing counselling and referrals.

Qualifications

  • Bachelor's degree in Social Work, Psychology, or related field.
  • Proven ability to coordinate assessments, care plans and service delivery.
  • Experience working with diverse stakeholders and caregivers.

  • Conduct psychosocial assessments and develop person-centred care plans.
  • Review and monitor Individual Support Plans with the multidisciplinary team.
  • Provide case management services including counselling and referrals.
  • Conduct home visits to assess living environments and needs.
  • Liaise with caregivers, hospitals, schools and community partners to ensure continuity of care.
  • Organise case conferences and multidisciplinary meetings.
  • Monitor progress and document outcomes and actions.
  • Maintain confidential client documentation and reports.

Job description

The Case Manager is responsible for coordinating person-centred case management services for clients and their caregivers. The role oversees client admissions, casework interventions, care planning, caregiver support, multidisciplinary coordination, and collaboration and internal and external stakeholders to ensure clients receive quality and holistic care.


Core Responsibilities


  1. Coordinates the admission, enrolment, transfer and discharge process for clients, including means testing and subsidy applications through the HOMES system.

  2. Conducts comprehensive psychosocial assessments and develops person-centered care plans based on clients' needs.

  3. Reviews and monitors Individual Support Plans (ISP) together with the multidisciplinary team to ensure interventions remain relevant and effective.

  4. Provides case management services, including counselling, caregiver support, crisis intervention and appropriate referrals.

  5. Conducts home visits where required to assess clients' living environment, caregiver support and service needs.

  6. Liaises and coordinates with caregivers, hospitals, schools, SG Enable, MSF, social service agencies and community partners to facilitate continuity of care.

  7. Organises and facilitates case conferences, multidisciplinary meetings and caregiver meetings.

  8. Monitors clients' progress, evaluates intervention outcomes and recommends appropriate follow-up actions.

  9. Maintains accurate, timely and confidential client documentation, assessment reports and case records.

  10. Prepares and submits reports, claims and documentation required by management, SG Enable and relevant agencies within stipulated timelines.

  11. Supports incident management, safeguarding of vulnerable persons and follow-up actions relating to client welfare.

  12. Participates in audits, accreditation exercises, quality improvement initiatives and service evaluations.

  13. Supports centre programmes, organisational projects and any other duties assigned by the Centre Manager.


Core Job Requirements

Education



  • Bachelor's degree in Social Work, Psychology, or related field


Experience



  • Minimum 2 years of experience working in the sector preferred

  • IT-proficient and competent with the use of Microsoft Office and client-related system

  • Demonstrated skills in organisation, management and leadership

  • Knowledge of and ability to work effectively with the community and stakeholders

  • Excellent verbal and written communication skills

  • Ability to establish and maintain positive working relationships with staff, volunteers, and the public

  • Must be self-directed and work independently
