You will develop and refine care workflows, lead case management initiatives, and collaborate with clinical and operational stakeholders to support patient-centered care. The role involves assessing patient needs, coordinating interventions and referrals, and guiding junior staff.

Qualifications

  • Degree or qualification in Nursing, Social Work, Allied Health, or related discipline with 6–8 years healthcare experience in case management or care coordination.
  • Experience with multidisciplinary healthcare teams; leadership or programme development is an advantage.
  • Knowledge of acute, community, home-based, rehabilitative, or specialised medical services is an advantage.
  • Strong stakeholder management and communication skills; ability to work independently.

  • Participate in quality improvement, research, training, and community engagement initiatives.

  • Experience working with multidisciplinary healthcare teams, with leadership, supervisory, programme development, or service improvement experience being an added advantage.
  • Knowledge of acute, community, home-based, rehabilitative, psychological, specialised medical, or surgical care services is an added advantage.
  • Strong stakeholder management, communication, interpersonal, organisational, analytical, and problem-solving skills, with the ability to work independently in a dynamic healthcare environment.
  • Proficient in Microsoft Office applications; familiarity with healthcare information systems is an added advantage.
  • Proficiency in local languages and dialects is an advantage for liaising with local languages-speaking and dialects-speaking patients, caregivers, and stakeholders.
