Job summary

Club HEAL is seeking a Social Work Associate to engage clients and caregivers through cognitive and reminiscence activities under the supervision of the Team Lead. The role involves coordinating CREST activities and ensuring smooth service delivery in a community setting.

You will provide administrative support, assist in outreach events with community partners, manage data and quarterly reports, and maintain data protection standards.

Qualifications

• Diploma in social work, counselling, psychology or other fields related to healthcare and community care.
• 2-3 years relevant working experience in healthcare/community care.

Responsibilities

• Engage clients and/or caregivers through meaningful activities such as cognitive stimulation, reminiscence, memory and linguistic activities.
• Provide administrative or logistical support to the team for CREST programme activities and data collation.
• Coordinate outreach and engagement events with community partners; prepare and submit quarterly CREST reports; ensure data protection adherence.
• Prepare resource materials for clients and caregivers; perform other duties as assigned by Reporting Officer.
