Job summary

Qualifications

• Diploma in Social Work or equivalent required.
• Minimum two years in social services, eldercare, or related fields.
• Proficient in documentation and reporting software.
• Valid Class 3 driving licence and ability to travel between locations.
• Ability to work independently and in emotionally sensitive situations.

Responsibilities

• Coordinate service arrangements for deceased elderly members with families and internal teams.
• Guide families through the process with clear information and practical support.
• Provide empathetic support while adhering to organisational guidelines.
• Plan and coordinate memorial services and remembrance events.
• Maintain accurate case records for reporting and compliance.
• Participate in rotating hotline duties to handle inquiries.
• Support junior staff and volunteers to ensure smooth operations.
• Suggest workflow improvements to services.
• Reach out to hospitals, nursing homes, and partners to build referrals.
• Assist with administrative tasks as assigned.
