Job summary

Qualifications

• Min Diploma/Degree in Social Work/Psychology/Counselling
• At least 1 year of relevant healthcare industry experience
• Good interpersonal and problem-solving skills
• Proficient in English and able to converse in dialects or other languages
• Office Hours: 5 days work week

Responsibilities

• Create personalised care plans tailored to residents' needs
• Implement care plans and record residents' progress
• Design and organise programs and activities addressing care requirements
• Prepare and submit review reports and monthly interview records every 6 months
• Collaborate with stakeholders to coordinate comprehensive resident care
