Job summary

CHIEF RECRUIT PTE. LTD. is seeking a healthcare-focused professional to support the clinical team, advise on subsidy schemes, and process renewals.

You will handle onboarding, initial assessments, and subsidy applications while coordinating with Case Managers and Social Workers. Your role includes documenting interactions and supporting client and caregiver services. You will engage in home visits when needed, assist with events, and contribute to monthly reporting.

Qualifications

• Diploma or advanced diploma, preferably in a healthcare-related discipline.
• Minimum 1–2 years of relevant experience in healthcare or social services.
• Compassionate and patient with strong empathy and active listening.
• Team player with interpersonal and communication abilities.
• Familiar with healthcare and social services landscape.

Responsibilities

• Support the clinical team in advising clients on financial assistance schemes and subsidies.
• Assist administrative processes to facilitate service delivery to clients and caregivers.
• Manage onboarding of new clients, including scheduling initial assessments and subsidy applications.
• Provide initial assessment support for client and caregiver needs under supervision.
• Coordinate access to community services to help clients and caregivers cope.
• Provide information, guidance, and referral support to clients and caregivers.
• Conduct joint home visits with Case Managers or Social Workers when required.
• Support planning and execution of client and caregiver related events or projects.
• Participate in case discussions with the clinical team.
• Prepare, consolidate and monitor monthly reports and statistics.
• Ensure timely and accurate documentation in digital systems and physical records.
• Perform other duties as assigned by the reporting officer.
