Job summary

PeopleSolve Pte. Ltd. is seeking a proactive Registered Nurse to deliver home-based nursing care in the community, perform assessments, monitor conditions, and provide appropriate interventions.

You will manage wound and surgical care, OPAT, paediatric nursing, phlebotomy, and a range of procedures, educating caregivers and working independently with sound clinical judgement in Singapore.

Qualifications

• Registered Nurse with SNB registration and Practising Certificate.
• Experience in community/home care settings is required.
• Competency in OPAT, wound/surgical care, palliative and paediatric nursing, phlebotomy, catheter and NG tube care, PICC line management.

• Provide home-based nursing care and clinical support to patients in the community.

Job description

About the Role

• Provide home-based nursing careand clinical support to patients in the community.


• Conduct nursing assessments, monitor patients' conditions and provide appropriate nursing interventions.


• Perform and manage wound and surgical care, including post-operative care.


• Provide palliative and end-of-life nursing care where required.


• Provide OPAT (Outpatient Parenteral Antimicrobial Therapy) and related nursing care.


• Provide paediatric nursing care for patients requiring home-based support.


• Perform phlebotomy and other nursing procedures in patients' homes.


• Perform and manage procedures including: Urinary catheterisation and catheter care, stoma care and stoma bag changes, NG tube insertion and management, PICC line care and management, etc.


• Educate patients and caregivers on medication, treatment plans, procedures and ongoing care needs.

What We Are Looking For

• Registered Nurse with a valid Singapore Nursing Board (SNB) registration and Practising Certificate.


• Relevant clinical experience in community nursing, home care, hospital or other relevant healthcare settings.


• Demonstrated competency and practical experience in: OPAT, wound and surgical care, palliative care, paediatric nursing, phlebotomy, catheter care, stoma care, NG tube management, and PICC line care and management.


• Strong experience in workplace injury management, health surveillance and employee health programmes.


• Valid BCLS, CPR and AED certification.


• Good clinical judgement and able to work independently.
