Job description

Our Homecare Nurses deliver clinical care to patients in a home setting under the Mobile Inpatient Care @ Home programme. This role requires strong clinical competency, independence, and the ability to manage patients safely outside of a traditional hospital environment.

Key Responsibilities

Clinical Care (Primary Role)

Core Clinical Responsibilities

• Vital signs monitoring and escalation of abnormalities
• IV therapy (PICC, CVC, peripheral)
• Physical assessment and patient evaluation

Procedural Skills

• IV cannulation and venipuncture
• Central line management
• Wound care, lines and drainage management

Communication & Documentation

• Communicate patient conditions effectively with internal teams and external stakeholders (e.g. hospital partners)
• Maintain accurate, timely documentation in clinical management systems
• Provide timely and clear handover updates where required
• Build and maintain good rapport with patients, caregivers, and stakeholders through professional and empathetic communication

Screening & Clinic Support

• Perform home-based health screening and baby vaccinations for corporate clients
• Support clinic/screening centre operations when required. Skills involved include: venipuncture, ECG, Treadmill ECG operation, administering vaccinations, vision acuity tests.

Requirements

Qualifications & Certification

• Valid registration with the Singapore Nursing Board (SNB) as a Registered Nurse
• Current BCLS + AED certification

Experience

• Prior experience in hospital settings preferred
• Experience in IV therapy, cannulation, and acute care management
• Exposure to homecare / community care settings is an advantage
• Strong clinical judgment and ability to work independently
• Ability to manage high-pressure situations and respond to emergencies
• Good communication and interpersonal skills
• Competent in clinical documentation systems (EMR experience preferred)

Operational Requirements

• Willing to travel islandwide for home visits (transport provided)
• Comfortable working in home environments and handling patients independently
• Able to carry medical bag required for home visits (hand-carry luggage size)

Working Hours

• Shift-based role between 7:00 AM to 12:00 midnight, includes morning, split, and night shifts
• 5-day work week, with rotating weekends and public holidays
