Job summary

RN Care Pte. Ltd. in Singapore seeks a registered nurse to perform biopsychosocial and environmental assessments, plan and evaluate person-centred nursing care, and communicate plans with patients and families.

The role covers wound care, medication management, and continence care while upholding patient dignity, safety and confidentiality. Collaboration with multidisciplinary teams and emergency protocol initiation are expected.

Qualifications

• Must have a Diploma in Nursing.
• Registered with the Singapore Nursing Board.
• Minimum 3 years' experience in acute and/or community hospital/care setting.

Responsibilities

• Perform biopsychosocial and environmental assessments to identify strengths and needs of clients, families and/or caregivers.
• Plan, develop and evaluate person-centred nursing care plans and recommend modifications according to changing needs.
• Communicate care plans and address concerns from patient and family.
• Implement and evaluate care interventions (e.g., wound care, medication management, continence care) with emphasis on safety, dignity and confidentiality.
• Manage patient workload through timely planning and scheduling for designated groups.
• Build therapeutic relationships to empower clients and caregivers for self-management.

Job description

Perform biopsychosocial and environmental assessments to identify strengths and needs of clients, families and/or caregivers.

Plan, develop and evaluate person-centred nursing care plans and recommend appropriate modifications according to client's changing needs. Communicate care plans and address the concerns from patient and family.

Implement and evaluate care interventions(s) e.g. wound care, medication management, continence care, ensuring at all times:

• Dignity of the patients
• Patient's safety
• Confidentiality of patients' information
• Advocacy on behalf of the patients

Practice safety and infection control precautionary measures.

Manage patient workload of a designated patient group through timely planning and scheduling based on the client's needs.

Build therapeutic relationships and empower the clients, families and/or caregivers for self- management through health promotion, caregivers training and motivational techniques.

Coordinate with multidisciplinary team for care integration and transition through:

• Participating in Multidisciplinary rounds
• Making referral to other members of the healthcare professional team (doctor, social welfare executive, etc.) when required
• Assisting clients to navigate the healthcare system

Initiate emergency protocols when necessary.

Discuss / escalation to Supervisor and/or Care team on any problems encountered in patient's condition and/or in the handling of patients and/or care-givers/family members.

Requirements
