Qualifications

• Bachelor degree in Healthcare/Public Health/Business/Social Sciences or related studies required.
• 3-5 years of working experience in community healthcare operations/programme management/population health.
• Hands-on experience in programme implementation, project planning and process improvement.
• Proficient in Excel and PowerPoint, with data analysis, reporting or dashboards.
• Able to track KPIs, programme outcomes and operational performance.
• Experience in budget tracking, procurement, vendor coordination or resource planning will be advantageous.
• Strong stakeholder management experience across healthcare, community care or social service partners.
• Exposure to UAT, digitalisation or system/process implementation is a plus.
• Able to manage multiple projects and deliverables in a fast-paced environment.

Responsibilities

• Lead end-to-end implementation of community healthcare programmes from planning to evaluation.
• Coordinate delivery with public hospitals, community care partners, primary care providers and social service agencies.
• Translate objectives into operational plans, workflows, and timelines.
• Coordinate outreach, events and programme operations for smooth delivery.
• Manage day-to-day programme needs including resources, vendors, logistics, budgets and operations.
• Track participation, service utilisation and KPIs to monitor progress.
• Gather stakeholder feedback and identify opportunities to improve delivery and patient/community experience.
• Support development of reports, dashboards and presentations for management and stakeholders.
• Build strong relationships with internal teams and external community partners.
• Identify operational gaps and drive process improvements for efficiency and accessibility.
• Support new initiatives, pilots and population health programmes as required.
