Muhammadiyah Welfare Home in Singapore seeks a dedicated professional to provide casework and counselling support to children and youths-at-risk, working closely with families to strengthen relationships and guide positive change through holistic care, intervention and support.

You will manage casework from admission to discharge, complete assessments, develop ICPs, provide counselling, maintain records, coordinate with agencies, and prepare reports for authorities such as MSF and Youth Court.

Qualifikationen

• Degree in related field.
• 1–2 years of relevant experience in casework and counselling.
• Strong interest in working with children and youths-at-risk.

Aufgaben

• Complete required assessments on residents and families using appropriate tools to inform intervention plans.
• Assess residents’ needs, strengths and risks; develop and review ICP.
• Provide counselling and interventions for residents and families, including planning and executing counselling sessions.
• Maintain accurate case records, reports and updates in relevant systems.
• Coordinate with internal teams and external agencies to support residents’ progress and family outcomes.
• Prepare and present reports to relevant authorities (e.g. MSF, Youth Court).
• Participate in case discussions, meetings and supervision sessions.
• Support any additional duties or projects as assigned.
