Job summary

Singapore Anglican Community Services (SACS) is seeking a community-focused case manager to deliver person-centered services. You will perform intake assessments, develop care plans, conduct home visits, provide counselling, and coordinate services with the care network to promote client well-being.

Working under supervision, you will engage in outreach activities, health talks, and screening initiatives while supporting volunteers and caregivers.

Qualifications

• Bachelor’s Degree preferably in Social Work, Psychology, Sociology, Gerontology or Counselling.
• 2 - 3 years relevant experience, candidates without experience are welcome to apply.
• Have a passion to work in a community-based setting; possess good knowledge of healthcare and social services;

Responsibilities

• Carry out case management work and apply intervention strategies under supervisor guidance.
• Involve in community outreach and public education initiatives; support volunteer management initiatives.
• Provide basic emotional support to caregivers and befriending to clients.
• Maintain case progress notes, home visit logs, and case documentation in timely manner.
• Develop, review, and update intervention strategies with supervisor.
• Participate in case reviews, supervisions, and conferences.
• Coordinate care networks with stakeholders within service boundaries.
• Assist outreach initiatives like community mental health screening and health talks.
