Job summary

AKG Group in Singapore is hiring a Case Manager to coordinate person-centered case management services for clients and caregivers. You will oversee admissions, care planning, and multidisciplinary coordination to ensure holistic care.

Responsibilities include home visits, liaising with hospitals and social service agencies, organizing case conferences, maintaining documentation, and supporting quality improvement initiatives within the Centre. Minimum 2 years' experience preferred.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field.
• Minimum 2 years of experience in the sector preferred.
• IT proficiency with Microsoft Office and client-related systems.
• Excellent verbal and written communication; ability to work with stakeholders.

Responsibilities

• Coordinates admissions, enrolment, transfer and discharge processes.
• Conducts psychosocial assessments and develops person-centered care plans.
• Reviews and monitors ISP with multidisciplinary team.
• Provides case management, counselling, caregiver support and referrals.
• Performs home visits to assess living environment and service needs.
• Liaises with caregivers, hospitals, SG Enable, MSF and other agencies.
• Organises case conferences and multidisciplinary meetings.
• Monitors progress, evaluates outcomes and follows up actions.
• Maintains confidential client documentation and reports.
