Job summary

PERSOL SINGAPORE PTE. LTD. is seeking a coordinator to collect and assess social and financial information about residents prior to financial assistance applications.

You will act as the main liaison for residents, families and healthcare institutions to coordinate referrals and process applications. Responsibilities include ensuring timely submissions for schemes like Medifund and IDAPE, maintaining case files, updating databases, handling enquiries, and overseeing transfers and discharge

Qualifications

• GCE N/O Levels or equivalent qualification.
• Experience in social work, social services, case management or related field preferred.
• Healthcare or community care sector experience advantageous.
