Job summary

Methodist Welfare Services is seeking dedicated staff to provide comprehensive eldercare in a Centre-based setting. The role includes clinical care, cognitive monitoring, medication reminders, personal hygiene support, and assistance with daily living activities.

You will engage clients through structured activities, ensure safety in transfers, and document care in Salesforce. Collaboration with qualified healthcare professionals and adherence to SOPs are essential.

Responsibilities

• Monitor clients' vital signs and weight; record and report deviations
• Observe cognitive levels and assist with dementia-related interventions
• Provide medication reminders and observe the 4 rights
• Assist with personal hygiene and grooming for clients with mobility issues
• Support oral feeding and assist with feeding where needed
• Assist safe tube feeding when required
• Monitor dietary adherence and nutrition intake
• Assist with ADLs and safe client transfers and mobility
• Conduct structured activities and document engagement in Salesforce
• Engage clients with person-centered care approaches
• Support emergency response and crisis management with stakeholders
• Manage relationships with stakeholders and support volunteers and students
• Maintain records in Salesforce and comply with SOPs and MOH requirements
• Prepare the centre for audits

Job description

Roles & Responsibilities

1. Clinical Care
   a. Monitor client’s vital signs and weight by measuring, recording and reporting the data gathered
   b. Report and assist with intervention for any deviation from baseline
   c. Develop and review ICP for clients every 6 month

2. Monitor Cognitive Level in Clients
   a. Monitor cognitive level in clients and participate in developing appropriate intervention in consultation with qualified healthcare professionals
   b. Observe for any change in behavior which may be possible signs of Dementia
   c. Initiate simple strategies to de-escalate onset of episode

3. Medical Assistance
   a. Communicate with clients in relation to remind them to take their pre-packed oral medication and or applying prescribed topical medication in accordance
   b. To observe the 4 rights (client, quantity, route, time)
   c. Observe clients for side effects

4. Provide Personal Hygiene Support
   a. Provide personal hygiene and grooming support, including skin care, for clients with mobility and ambulation problems

5. Perform Oral Feeding for Client
   a. Support client with or without swallowing difficulties in oral feeding
   b. Encourage client participation to maximize independence

6. Assist on Tube Feeding
   a. Support client by performing a safe tube feeding process

7. Monitor Adherence to Diet Plan
   a. Observe, identify, record and report client’s nutritional intake and adherence to diet plan

8. Activities of Daily Living (ADLs)
   a. Provide care and assistance in ADLs accordance with their care plans
   b. Recognize boundaries and consult relevant personnel when needed
   c. Perform client’s transfers safely and provide feedback on transfer status
   d. Maneuver client’s wheelchair with good safety awarenes
   e. Identify and understand needs and concerns of clients with different physical abilities

9. Conduct Structured Activities
   a. Conduct and supervise one-to-one and group structured activities
   b. Conduct physical activities suitable with the right challenge activities for clients with different physical abilities
   c. Conduct activities suitable for persons with Dementia
   d. Accurate documentation of client’s engagement in Salesforce

10. Engage Clients with Person Centered Care (PCC) Approach
    a. Able to use PCC as a guiding principle when engaging clients

11. Emergency Response and Crisis Management
    a. Able to support responsibilities as prescribed in Emergency response and crisis management plans and work with external and internal stakeholders to conduct periodic reviews and ensure emergency preparedness

12. Stakeholder Management
    a. Able to Support and manage constructive and positive relationships with stakeholders (internal and external)
    b. Provide support to volunteers, interns and students attached to the Centre

13. Administrative Management
    a. Ensure that all records of clients in Salesforce are updated
    b. Understand and comply with SOPs and MOH service requirement
    c. Ensure that Centre is ready for audit

Qualifications, Competencies & Experience

• At least a GCE “N” level / HMI / WSQ related study in Healthcare

• At least 2-3 years of relevant experience in Eldercare

• IT savvy

• Must enjoy working with elderly with physical disabilities and/or dementia in Centre-based setting

• Able to multi-task and work in a physical environment that is dynamic

• Able to work with a team with people of all levels and adaptable to the potential need of change of job tasks
