Job summary

NHG Health’s Population Health division seeks a Senior Executive / Assistant Manager for Care Transition Services. You will support planning, operations, implementation and management of care transition programmes across NHG Health hospitals, primary care and community care sectors.

You will work with Right-Siting Officers and clinical teams to facilitate timely transitions from hospital to primary care and community providers, helping integrate care pathways and resources for continuity of care.

Qualifications

• Degree holder with good interpersonal and communication skills.
• Fresh graduates welcome; related work experience advantageous.
• Experience in community care, communications, project management or business is a plus.

Responsibilities

• Provide secretariat support for departmental, divisional, and cross-functional meetings.
• Plan, coordinate, and manage projects, events, and institutional initiatives.
• Support onboarding, orientation, and training of new staff.
• Build and maintain effective working relationships with internal and external stakeholders.
• Undertake other duties and responsibilities as assigned by the supervisor.
