NHG Health is seeking a Care Transition Coordinator in Singapore to bridge acute hospitals and community care settings. You will ensure appropriate placement for discharged patients and maintain robust links with General Practitioners, Family Medicine Centres and Polyclinics.

You will work with clinicians and clinic managers to identify suitable patients, facilitate seamless transfers, and provide information to patients while coordinating access to local primary and community care resources.

Qualifications

• Diploma/NITEC in nursing or health sciences as required
• Ability to maintain accurate and timely documentation
• Fresh graduates welcome and considered for placement

Responsabilités

• Coordinate discharge planning and patient placement to primary/community care.
• Collaborate with clinicians, clinic managers and multidisciplinary teams.
• Produce regular administrative reports on patient indicators and care outcomes.
