• Minimum 5 years of clinical or care assessment experience in eldercare or community health.
• Certified InterRAI Assessor or willing to certify immediately.
• Strong cross-division collaboration and change-management experience.
• Ability to scope workload and supervise junior staff.

Responsibilities

• Establish centralized assessment guidelines and supervise up to 4 assessors.
• Streamline internal onboarding and care handoffs.
• Drive the One Care Plan strategy across NTUC Health.
• Ensure compliance with MOH and governance frameworks.
• Lead efforts to identify gaps and improve care journeys.
