Recruit Express Pte Ltd in Singapore is seeking a dedicated social worker to conduct assessment, planning, and intervention for cases within the community.

You will engage clients, develop care plans, coordinate services islandwide, document outcomes, and support caregivers. Travel is required; transport is reimbursed.

Qualifications

• Minimum graduate diploma or degree in social work or relevant field
• Registered social worker with SASW is an advantage

Responsabilités

• Conduct assessment, planning and intervention for cases in consultation with the program lead
• Engage clients and build rapport
• Develop, implement and review individual care plans and intervention strategies
• Perform needs assessment of clients and caregivers
• Advocate for clients’ needs
• Coordinate services with other community resources for clients
• Document case notes and progress
• Manage case referrals
• Participate in case conferences/MDT meetings
• Monitor delivery of coordinated services and evaluate outcomes
• Collaborate with internal units and other agencies for multi-disciplinary assessments
• Refer and coordinate care services to support community living
• Provide information on assistance schemes or care options to caregivers
• Support discussions on advance care planning and end-of-life preferences
• Support administrative functions and maintain accurate records

Location: Bukit Merah but required to travel islandwide for cases (Transport reimbursed)

Job Scope:

• * In consultation with the program lead, conduct assessment, planning, and intervention for cases


• * Engage clients and build rapport


• * Develop, implement and review individual care plans and intervention strategies


• * Perform needs assessment of clients and caregivers


• * Advocate for clients’ needs


• * Coordinate services with other community resources on the client’s behalf


• * Case documentation


• * Manage case referralsi>
  
• * Conduct and participate in case conferences/MDT meetings


• * Monitor delivery of coordinated services


• * Document and evaluate case outcomes


• * Adopt a case management approach when working with clients


• * i.e. collaborate with internal business units and other agencies/specialists to provide multi-disciplinary assessments and monitoring of clients


• * Refer and coordinate care services for clients to ensure they are adequately supported to continue living in the community


• * Provide information on assistance schemes or care options to help caregivers make informed decision about care and future planning


• * Support discussions on advance care planning and end-of-life preferences


• Support Administrative Functions


• * Maintain accurate records and update progress reports and BumbleBee timely

Job Requirements:

• Min. Graduate Diploma/ Degree in social work or any relevant field


• Registered social worker with SASW will be an added advantage
