Apex Harmony Lodge in Singapore seeks an experienced nurse leader to guide care for residents, mentor nursing staff, and drive dementia-focused practices. The role involves clinical oversight, education, and collaboration with multi-disciplinary teams to sustain high-quality, person-centered care.

The ideal candidate has extensive LTC or geriatrics experience, strong leadership, and a commitment to safety, dignity, and family education.

Qualifications

• Experience in long-term care (LTC) or geriatrics preferred.
• Proven leadership and coaching abilities.
• Strong clinical assessment and documentation review skills.
• Ability to educate and counsel residents and families.

Responsabilités

• Timely recognition and escalation of clinical deterioration.
• Support residents and families through education and decision-making.
• Promote safety, dignity, and resident well-being.
• Review and oversee care plans and documentation.
• Lead clinical audits and quality improvement initiatives.
• Promote medication safety and infection prevention.
• Act as clinical expert and role model for staff.
• Ensure quality, continuity and compliance.

You will be responsible for:

• Timely recognition, escalation, and management of clinical deterioration and emergencies
• Supporting residents and families through education, counselling and decision-making
• Promoting safety, caring and dignified environments where residents can live well
• Reviewing and overseeing clinical assessments, care plans and documentation
• Leading clinical audits, incident reviews and quality improvement initiatives
• Promoting medication safety, infection prevention and resident safety
• Championing person-centred and evidence-based clinical care
• Serving as a clinical expert and role model
• Ensure quality, continuity and compliance
