Woodlands Hospital, a next-generation healthcare system in Singapore, seeks an Assistant Nurse Clinician to deliver high-quality patient care and support nursing excellence within the department.

Qualifications

  • Diploma or Degree in Nursing is required and registration with the Singapore Nursing Board is essential.
  • Minimum 7–10 years of clinical nursing experience and demonstrated leadership in clinical initiatives.
  • Strong clinical assessment, communication, and stakeholder management capabilities.

Responsibilities

  • Function as primary nurse for patients with complex care needs and manage holistic nursing care.
  • Identify at-risk patients and coordinate follow-up after discharge.

We are looking for a dedicated and clinically competent Assistant Nurse Clinician to play a pivotal role in delivering high-quality patient care while supporting nursing excellence within the department.

You will work closely with multidisciplinary teams, patients and caregivers to ensure safe, coordinated and person-centred care across the patient journey.

Key Responsibilities
Deliver Excellence in Patient Care
  • Function as the primary nurse for patients with complex care needs, ensuring comprehensive and holistic nursing management.
  • Assess, plan, implement and evaluate individualized care plans to achieve optimal patient outcomes.
  • Identify patients at risk of readmission and coordinate appropriate interventions and post-discharge follow-up.
  • Collaborate with internal and external healthcare providers to support seamless transitions from hospital to home or community care settings.
  • Lead and facilitate nursing rounds to promote patient-centred and evidence-based care.
  • Support nurses in addressing transitional care requirements and ensuring continuity of care upon discharge.
Be a Clinical Resource and Mentor
  • Provide clinical guidance and support to nursing staff in managing complex patient situations.
  • Mentor, coach and develop junior nurses through bedside teaching, nursing rounds and competency development activities.
  • Facilitate nursing education programmes and support continuing professional development.
  • Act as a preceptor, assessor or competency facilitator where required.
Lead Quality and Patient Safety Initiatives
  • Promote evidence-based nursing practice and drive quality improvement initiatives.
  • Identify clinical risks, gaps in practice and opportunities for service enhancement.
  • Educate and support staff in risk management and patient safety practices, including falls prevention, infection control, medication safety and pressure injury prevention.
  • Participate in audits, peer reviews and performance improvement projects to strengthen care delivery standards.
  • Monitor compliance with nursing policies, procedures and regulatory requirements.
Partner Across the Care Continuum
  • Collaborate actively with multidisciplinary teams to develop and implement integrated care plans.
  • Advocate for patients and caregivers to ensure their needs, preferences and goals are respected.
  • Support patient and family education initiatives to enhance health literacy and self-management capabilities.
  • Strengthen partnerships with healthcare and community providers to improve patient outcomes beyond hospital care.
Drive Professional and Service Development
  • Contribute to the development and review of nursing policies, care guidelines and patient education materials.
  • Assist in identifying departmental learning and development needs and support competency frameworks.
  • Participate in projects and initiatives that enhance nursing practice, service delivery and workforce capability.
  • Maintain professional competence through continuous learning and active participation in continuing nursing education programmes.
Demonstrate Leadership and Accountability
  • Lead by example through professional conduct, clinical expertise and sound decision-making.
  • Supervise and coordinate nursing resources effectively to ensure safe and efficient operations.
  • Promote a positive, collaborative and patient-focused culture within the department.
What You Bring
Qualifications & Experience
  • Diploma or Degree in Nursing from a recognised institution approved by the Singapore Nursing Board.
  • Registered Nurse with a valid Practising Certificate issued by the Singapore Nursing Board.
  • At least 7 to 10 years of relevant clinical nursing experience.
  • Demonstrated competence in managing complex patient care and leading clinical initiatives.
