Job Summary

The Assistant Care Manager assists in providing operational leadership and care coordination for the Active Ageing Centre (AAC) to support seniors ageing well in place with dignity, in alignment with Ministry of Health (MOH) and AIC Age Well SG objectives. This role oversees day-to-day programme execution, client assessment, case management, and team supervision, while ensuring compliance with funding, reporting, and quality assurance standards.

Key Responsibilities
Operational Leadership & Governance

Support the Senior Care Manager in executing daily centre operations and annual strategic plans aligned with MOH and AIC service agreements.

Maintain adherence to regulatory standards, funding conditions, safety protocols, and emergency/crisis management procedures.

Assist with operational reporting, budget monitoring, procurement, and maintaining audit readiness.

Programme Delivery & Care Coordination

Oversee the design, scheduling, and execution of active ageing programmes across core domains: Physical, Cognitive, Social, Learning, and Volunteerism.

Lead client intake, screening (e.g., CST/interRAI assessments), case coordination, and early risk detection for seniors with emerging social, physical, or mental health needs.

Facilitate integration with Community care partners, healthcare institutions, and social service partners to enable smooth care pathways.

Track programme attendance, participant satisfaction, and outcomes to recommend continuous service improvements.

Community Engagement & Outreach

Drive community outreach initiatives, block visits, and neighbourhood engagement activities to achieve senior recruitment targets and Ground-Up Initiatives (GUIs).

Support volunteer management processes, including volunteer recruitment, orientation, deployment, and recognition.

Build and maintain positive working relationships with community partners and local agencies.

Team Supervision & Staff Development

Assist in supervising, guiding, and mentoring junior programme staff and care staff to foster a high-performing and collaborative team culture.

Provide practice guidance and support staff training and development needs.

Data Management & Administration

Ensure accurate documentation of client data in relevant management databases and client management systems in strict compliance with PDPA guidelines.

Compile daily operational metrics, attendance logs, and evaluation reports for internal leadership and external agencies (AIC/MOH).

Requirements
Qualifications

Degree or Diploma in Social Work, Gerontology, Psychology, Nursing, Community Development, or related Social Sciences / Allied Health disciplines.

Experience & Skills

Experience: Minimum 2 to 5 years of experience in eldercare, community development, or healthcare settings, with at least 1-2 years in a supervisory or senior operational role (preferably within an AAC or eldercare centre).

Sector Knowledge: Familiarity with MOH/AIC service guidelines, community screening tools, and eldercare ecosystems.

Competencies: Strong team coordination, case management, problem-solving, and communication skills.

Technical Proficiency: Proficient in Microsoft Office and client/data management software tools. Experience with interRAI assessments is an advantage.

Languages: Proficiency in English and Mandarin (and/or local dialects) is preferred to engage effectively with senior populations.
