Job summary

PERSOL is seeking an Assistant Care Coordinator in Singapore to support patients receiving home care. You will coordinate with healthcare teams, patients and caregivers, monitor patient conditions, provide updates, and contribute to care plans and education.

The role requires strong interpersonal skills and the ability to work in a fast-paced environment. You will handle inquiries, document care progress, and assist with referrals and telehealth monitoring.

Qualifications

• Diploma in a healthcare-related field preferred.
• Basic computer proficiency.
• Comfortable with shift work including evenings, weekends and public holidays.
• Able to work independently in a fast-paced environment.
• Customer-focused with strong interpersonal and communication skills.
• Good teamwork with some leadership potential.
• Prior healthcare or hospital experience is an advantage.

Responsibilities

• Coordinate with nurses and healthcare teams to support patient care at home.
