Job summary

PERSOL Singapore is seeking an Assistant Care Coordinator to support patients receiving home care by coordinating with healthcare teams, patients, and caregivers. The role involves monitoring patient conditions, providing updates, and delivering education to ensure comprehensive care at home.

You will participate in care planning, document progress, and attend care team meetings. The position requires flexibility for shifts, including evenings, weekends, and public holidays, and values

Qualifications

• Diploma in a healthcare-related field preferred.
• Basic computer proficiency.
• Comfortable to work shifts, including evenings, weekends, and public holidays.
• Able to work independently in a fast-paced environment.
• Customer-focused with strong interpersonal and communication skills.
• Good teamwork skills with some leadership potential.
• Prior healthcare or hospital experience is an advantage.

Responsibilities

• Conduct pre-admission screening for patients and caregivers.
• Coordinate with nurses and healthcare teams to support patient care at home.
• Monitor patients' condition and report any concerns or changes.
• Handle patient and caregiver enquiries and provide care plan updates.
• Develop and document care plans, patient progress, and treatment records.
• Attend care team meetings and contribute to patient care discussions.
• Arrange referrals and coordinate with community healthcare services.
• Monitor patients' vital signs through telehealth systems and elevate issues when needed.
• Provide basic patient education and health coaching on areas such as diet, exercise, and wellness.
• Ensure necessary equipment and supplies are available for patient care.
• Support quality improvement projects and perform other assigned duties.
