Job summary

You will coordinate with nurses, caregivers and providers, maintain accurate documentation, and participate in quality improvement initiatives to enhance patient outcomes and continuity of care.

Qualifications

• Diploma in Health Promotion or related healthcare discipline.

As a Assistant Care Coordinator (Healthcare), you will be responsible for the following duties:

• Conduct pre-admission screening and assess patients’ care needs.
• Support nurses and healthcare professionals in providing holistic home-based care.
• Monitor patients’ conditions and identify concerns requiring escalation.
• Develop and update individualised care plans based on patient needs.
• Communicate with patients and caregivers regarding care plans and enquiries.
• Monitor vital signs remotely using telehealth systems and digital platforms.
• Provide basic health education and lifestyle coaching on diet, nutrition and exercise.
• Maintain accurate documentation of patient care, progress and treatment plans.
• Coordinate with healthcare providers, community resources and vendors to ensure continuity of care.

Requirements:

• Minimum Diploma in Health Promotion or healthcare-related disciplines.
